Privacy Policy

Frayed Edges' commitment to your privacy:

Frayed Edges is dedicated to maintaining the privacy of your personal health information. We are also required by law to do this. These laws are complicated, but we must provide you with important information. This form is a shorter version of the full, legally required NPP, a copy of which you can request for your reference. However, we cannot cover all possible situations. Please contact Frayed Edges with any questions or concerns.

Frayed Edges uses information about your health, which we get from you or others, mainly to provide you with treatment, to arrange payment for our services, or for other business activities that are called, in the law, and/or health care operations.  No tracking technologies are placed on patient portals or other pages where protected health information is collected, and no identifiable health information is disclosed through any vendors.

As a client, we ask you to sign a Consent to Use and Disclose Your Personal Health Information form to let Frayed Edges use and share your information with your consent in accordance with the federal privacy laws. Federal law requires your agreement/consent in order for us to provide services to you.

While Frayed Edges will keep your health information private, there are some situations that do not happen very often, but are described in the longer version of our privacy policies, which can be provided upon request.

Frayed Edges does not share mobile numbers, text messaging originator opt-in data, or consent with any third parties or affiliates for marketing or promotional purposes.  If you choose to utilize email or text messaging services, text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the Text Message services.
